What are the documents required when a minor turns major?

1. The prescribed MAM form, duly completed in all respects. Note: The applicant’s (minor who has turned major) signature on the MAM form must be attested in the designated space by the parent/guardian whose signature is registered in the mutual fund’s records for the minor’s folio, or by a Notary or a Judicial Magistrate First Class (JMFC). Alternatively, the applicant’s signature can be attested by the unitholder’s banker in the prescribed format provided in Banks Declaration.
2. A copy of the applicant’s PAN card.
3. KYC acknowledgment or a duly completed KYC form.
4. A canceled cheque with the applicant’s name pre-printed or the latest bank statement/passbook.
5. Signature attestation by the banker in the Bank Declaration form (if not attested by the guardian, a Notary, or a JMFC in the MAM form).
6. A duly filled Nomination Form.
7. A fresh mandate for SIP, STP, or SWP in the prescribed format to ensure the continuation of these transactions, if applicable.
